CSLA Coaches and Sports by Season

Athletic Trainer

Certified Athletic Trainer – Sammie Jones, M.Ed. LAT, ATC, ITAT

Sammie started at CSLA in June 2023. Sammie is a board-certified athletic trainer, licensed in both Tennessee and Georgia. In 2022, Sammie was named the Tennessee Athletic Trainers’ Association Joe Worden Clinic/Professional Athletic Trainer of the Year! She is an ImPact Trained Athletic Trainer and holds multiple certifications in RockTape’s FMT classes. Sammie is originally from Littleton, Colorado. Prior to CSLA, Sammie received her bachelor’s in athletic training from Northern Kentucky University. She did a variety of clinical rotations during her time at NKU, including high school, college, and clinic rotations. After her time at NKU, she completed a graduate assistantship at Mercer University working primarily with Women’s Soccer and Cheer & Dance. While at Mercer, she completed her master’s in education. After Mercer, Sammie was at Chattanooga Central High School for 6 years before coming to CSLA.

Sammie is currently employed through Erlanger Health System who contracts coverage to all Hamilton Co High Schools. Sammie covers all high school sports at CSLA. She works closely with the team physicians and the physician’s assistants at Erlanger Sports Medicine.

IF YOUR CHILD IS INJURED:

  • Sammie will evaluate all injuries that occur at practice and/or games that she is aware of.
  • If your child comes home complaining about an injury that is not a medical emergency, let their coach or Sammie know so she can evaluate it and make a referral if necessary.
    • Athletic trainers can save you money in the long run by being able to evaluate, rehabilitate, and treat a variety of sports injuries.
  • If you take your child to a doctor, please make sure your child has a doctor’s note to turn into their coach/Sammie, so we are aware of any restrictions/physician clearance.

During Fall season a Saturday morning walk-in injury clinic is offered from 8am-11am.

PHYSICALS

Student-Athletes need a new physical for each school year dated after April 15th. Athletes need to have all forms completed in order to participate in athletics.

Athletes and their parents should complete the registration for FinalForms and fill out all required forms online. All forms can be completed electronically with the exception of the Physical Examination form that is signed by the doctor. This form will need to be uploaded (either a picture or a scan is fine as long as it is readable) and will need to be turned into Sammie and/or their coach.

Final Forms



All CSLA High School and Middle School student-athletes are required to be registered in our online athletics paperwork system – FinalForms – per Hamilton County Schools.

FinalForms is an online forms and data management service that allows you to complete and sign athletic participation forms for your student-athletes online. FinalForms saves data from season-to-season and year-to-year, meaning that you will not need to enter the same information twice! You will only need to review your information once prior to each school year to verify the information is current & correct and sign all required forms.

Hints:
• If your student-athlete co-ops a sport at another school, please sign up for CSLA only and select those sports. It will link them to the co-op school for that sport only and CSLA for everything else.
• Bookmark https://hamiltoncounty-tn.finalforms.com or save it to your desktop on your mobile device or computer (do not Google search for it in the future as you will not go directly to the Hamilton County log in page).
• Remember your username and password! You will use the same account and sign in until your child graduates – no need to create a new account each year. However, if you forget your password from one
year to the next, you can enter your email address to reset your password.
• If you have multiple student-athletes, you will only need one parent account to register them. After you register the first, you will click “My Students” to register any additional children.
• Have required information available, such as medical history/health information, insurance company policy numbers, etc. This will make the registration time be more efficiently completed.
• When selecting sports, select all sports your child participates in or is planning on trying out for. It is easier for us to remove them from a roster than to add them later.
• Parents will need to fill in your student’s email address. FinalForms requires the students email address to complete their portion of the registration (e.g., signing their part of the forms). Your student will need
to check their email for information on how to complete their portion of the registration.
• Use the Parent Playbook to help you with the online registration process. Please note, once you register, you will receive an email to confirm your account, create a password, and then complete the required forms.
• Physicals must be dated after April 15th to be valid for the next school year.
• To register for a new school year before full rollover in June – click the blue box at the top to switch to the new year’s registration.

Thank you to all the athletes that attended tryouts. The numbers listed below have made the team and will report for practice beginning Wednesday, February 5, from 4:00-5:15. Parent meeting to begin at 5:15.

Policies

Middle and High school students have the opportunity to join CSLA sports teams. All sports follow the TSSAA and TMSAA guidelines. Participation is dependent upon the following:

  • All students must submit a physical exam form signed by a physician before the student tries out, practices, or participates. Forms are required each year and cannot be done earlier than April 15, 2023 to be eligible for the 2023-2024 seasons. Students below 6th grade are not eligible.
  • Students must meet all TSSAA and TMSAA eligibility requirements.
  • The athletic participation fee must be paid before a student is permitted to participate on the team. The athletic registration fee is $75 per child. If your child plays football or wrestling at Tyner Middle School or Central High School, they are responsible for the Tyner athletic fee in addition to our CSLA athletic fee.
  • Individual teams will charge a team fee as set by the coach. The team fees will be published by the coach at the beginning of each season.
  • Newly enrolled students who have previously participated in varsity level athletics may not participate the first year enrolled unless the Executive Director of TMSAA and/or TSSAA grants permission by approving a hardship.
  • Any student who repeats a grade is not eligible until he/she reaches the next higher grade, with the exception of students repeating the 8th grade. These students are not eligible to participate in the 9th grade.
  • In order to try out for an athletic team, students must be passing. If students are making up work that has not yet been entered into PowerSchool by their teacher, they may attend tryouts. However, the student may not make the team with an F.
  • Students with grades that drop to an F during the season have two weeks to show progress and effort to improve the F. If no progress/effort is made then the student will be benched. Tutoring will take precedence over practice or play during this probationary period.
  • Students who fail any subject in a nine-week grading period are not eligible to participate in athletics for the next nine-week grading period. Any student failing a subject in the final nine-week grading period must attend and satisfactorily complete summer school to become eligible for fall athletics in the upcoming school year.
  • Students who have 2 or more Fs are immediately benched until grades improve.
  • Student athletes must be present at school a minimum of 1⁄2 day to be eligible for practice or competition on that day.
